Shared Care

Shared Care initiatives are improving the coordination of patient care between family and specialist physicians. The relationship between family physicians (FP) and specialist physicians (SP) is fundamental to the delivery of effective health care, especially for more complex patient populations.

Princeton Access to Specialist Care

In an effort to support family physicians and to provide better access for Princeton patients, a Shared Care project was started in the Fall of 2013. Penticton specialists started holding outreach clinics in Princeton, eliminating some patient travel and providing family physicians with lunchtime CME learning opportunities.

The first phase of the project is focused on outreach clinics and supporting Princeton physicians. The second phase of the project, started early in 2015, is focused on efficiencies for patients when they travel to Penticton for appointments and tests.

Collaborating to improve health outcomes and the patient experience.

The Shared Care Committee is a joint committee of Doctors of BC and the Ministry of Health that works to improve health outcomes and the patient journey through the health care system. The committee was formed in 2006, as part of the Physician Master Agreement to address the growing need for coordination of patient care and best use of health care resources. Since then the SCC has helped more than 2,500 family physicians, 240 specialist physicians, and many other partners, to work together on over 240 projects across BC. These projects fall under ten initiatives as part of this joint Doctors of BC/Ministry of Health committee. They are as wide-ranging in scope and geography as they are in focus.

For more details about the ten initiatives please visit the SharedCare website.
